While some verification is important, delaying action until every data point is perfect slows the response and can cost opportunities. In contrast, acting promptly on timely, actionable intel keeps operations nimble and maintains the initiative. Choices that emphasize delaying actions or tying dissemination to unrelated cycles miss the urgency and dynamic nature of battlefield decisions.

When a service member’s ability to perform duties is compromised by a medical condition, a Medical Evaluation Board is initiated. This formal medical review brings together all relevant medical evidence—diagnoses, tests, functional limitations, and how the condition affects the ability to perform duties—to determine if the member is still fit for continued service. If the MEB concludes the member is unfit for duty, the case then moves into the disability evaluation process, which leads to a formal disability rating and disposition (such as retirement or separation with disability benefits). This differs from a Disability Evaluation System in that the DES is the broader program that manages disability processing, with the MEB being the specific medical step that assesses fitness for duty. The Personnel Reliability Program is about maintaining security and reliability of personnel, not diagnosing medical disability. A Fitness assessment, on the other hand, is a routine check of physical readiness, not a formal determination of long-term capability to perform duties due to disability.

Stratification describes the level of medical readiness and deployment potential for conditions needing specialist care. An ALC Y designation with a C2 stratification means the condition will likely require specialist medical care and referral within a year, but the service member can still be deployed or reassigned outside CONUS or to non-fixed environments if appropriate specialty care is available, or for short periods. This aligns with a scenario where deployment is possible with access to specialty care and for limited durations, rather than being non-deployable or fully unrestricted. A C3 would imply stricter limits and less deployability, C1 would imply full, unrestricted deployability, and Waiver Review is a different pathway used for waiver considerations, not the described stratification.

HIV testing results belong in the service-specific medical system of record. This is the official health record for a service member within that branch, ensuring the data stays with the member’s primary medical file and is accessible to authorized clinicians across DoD facilities within that service. It also supports proper privacy and medical governance. The DoD-wide Joint Medical Archive exists for cross-service access and long-term storage, but it is not the primary clinical record used for day-to-day care. Local clinic files and a national registry do not serve as the official, centralized source of record for the member’s HIV results.

A Sick Slip is a manual way to track a service member’s medical status and where they should reside (their quarters) when the electronic medical system isn’t available or the clinician is working with another branch of service. In these situations, the DD Form 689 is used to document the patient’s quarters location and related status because the usual digital records (ASIMS) can’t be accessed or a cross-service provider is involved. This form provides a temporary, paper record to ensure the member is cared for and properly supervised while the electronic system or cross-service coordination is resolved. It’s not for travel authorization, leave requests, or a basic medical report—those have different forms and purposes.
