About this Exam

The Certified Flexible Endoscope Reprocessor (CFER) certification is a prestigious credential designed specifically for healthcare professionals dedicated to the safe and effective reprocessing of flexible endoscopes.

This specialized role is critical in modern healthcare, as flexible endoscopes are complex, reusable medical devices used in numerous diagnostic and therapeutic procedures. Proper reprocessing is vital to prevent healthcare-associated infections (HAIs) and ensure patient safety.

The CFER certification, administered by the Certification Board for Sterile Processing and Distribution (CBSPD), validates that an individual possesses the specialized knowledge and technical skills necessary to handle decontamination, high-level disinfection, sterilization, storage, and maintenance of these intricate instruments.

This certification is designed for Sterile Processing Technicians, Endoscopy Technicians, and other GI laboratory personnel who already possess significant, hands-on experience in flexible endoscope reprocessing. Eligibility typically requires a minimum of 12 months of full-time employment performing these duties or a combination of educational courses and reduced experience. It is not an entry-level exam; it is a mark of professional mastery.

Achieving CFER status demonstrates your commitment to quality assurance, regulatory compliance, and the highest standards of patient care. It signifies to employers and colleagues that you are an expert in your field.

What the Course Entails and Exam Details

Preparing for the CFER exam requires a deep understanding of standard practices, manufacturer instructions for use (IFUs), and governing regulations. The exam does not have a mandatory "course" associated with it, but most candidates study the CBSPD’s official Flexible Endoscope Reprocessing Manual and other industry-standard guidelines.

The knowledge base covered by the CFER exam spans the entire lifecycle of a flexible endoscope. The core domains include:

  • Scopes and Accessories: Understanding the types, anatomy, functions, and unique challenges of various flexible endoscopes (e.g., colonoscopes, bronchoscopes, duodenoscopes).
  • Life Science: Basic microbiology, standard precautions, and the types of microorganisms relevant to endoscopy.
  • Handling and Cleaning: The critical steps of bedside pre-cleaning, transport, visual inspection, and manual/mechanical cleaning.
  • Reprocessing: Detailed knowledge of leak testing, high-level disinfection (HLD), chemical sterilization parameters, and rinsing procedures.
  • Rules, Regulations, and Safety: Adherence to OSHA, FDA, CDC, and EPA guidelines; chemical safety (SDS); and facility policies.
  • Documentation: Accurate record-keeping for cleaning cycles, HLD parameters, sterilizer loads, maintenance, and scope tracking.
  • Ethics: Professional conduct, patient rights, and the ethical responsibilities of a reprocessor.

What to Expect in the Final Exam

The CFER exam is a rigorous, standardized test that measures your competency against national standards. It is a computer-based examination (CBT) taken at an authorized testing center.

  • Exam Format: The examination consists solely of multiple-choice questions. Each question has four possible answers, with only one being correct. There are no practical or hands-on components.
  • Number of Questions: You can expect to encounter 100 scored multiple-choice questions. It is important to answer every question, as there is no penalty for guessing.
  • Time Limit: Candidates are allowed 2 hours (120 minutes) to complete the entire examination. This allows for approximately 72 seconds per question. Effective time management is key.
  • Passing Score: The CFER exam is scored on a scaled spectrum. A scaled score of 70 is required to pass the examination. Your raw score (the number of questions you got correct) is converted into this scaled score.
  • Rules: No study materials, cellular phones, or electronic devices are permitted in the testing room. You will need to provide valid photo identification. Scratch paper is usually provided by the testing center and collected afterward.

How to Study and Exam Centers

Preparation is everything when aiming for the CFER credential. Because eligibility requires experience, your daily work is your best primary resource, but you must supplement it with structured study to ensure your knowledge aligns with current national standards, not just your specific facility’s practices.

Actionable Study Strategies:

  • Study the Official Manual: Your most crucial resource is the official CBSPD Flexible Endoscope Reprocessing Manual. Read it thoroughly. Take detailed notes.
  • Review Guidelines: Become familiar with current guidelines from the Association for the Advancement of Medical Instrumentation (AAMI), specifically ANSI/AAMI ST91, and the Association of periOperative Registered Nurses (AORN).
  • Use Practice Exams: Take reputable CFER practice exams under timed conditions. This is the single most effective way to identify your knowledge gaps and familiarize yourself with the question formatting. Analyze every question you miss to understand the rationale behind the correct answer.
  • Create Flashcards: Create flashcards for critical information such as chemical contact times, leak testing steps, relevant regulations, and medical terminology.
  • Form a Study Group: Collaborating with colleagues who are also studying can provide motivation and different perspectives on complex topics.

Testing Centers:

The CFER exam is administered via computer-based testing at secure centers managed by CBSPD's testing partner (currently PSI Services). These authorized testing centers are located throughout the United States and internationally. Upon successful registration with CBSPD and payment of the exam fee, you will receive an authorization to test email, allowing you to schedule your exam appointment at a time and location most convenient for you within the designated testing window.
