The COMAT FBS exam focuses exclusively on the foundational biomedical sciences through an osteopathic lens. Unlike clinical COMATs, which cover single specialties, the FBS Comprehensive exam is a multidisciplinary assessment encompassing the broad spectrum of pre-clinical knowledge.

The curriculum and blueprint cover two primary dimensions: Body Systems and Disciplines.

The integrated content blueprint includes the following core areas:

• Body Systems: Cardiovascular and Hematologic; Endocrine and Metabolism; Gastrointestinal and Nutritional; Genitourinary/Renal; Integumentary; Musculoskeletal; Nervous System and Mental Health; Respiratory; and Human Development, Reproduction, and Sexuality.
• Disciplines: Biochemistry and Genetics; Foundational Anatomical Sciences (Gross Anatomy, Embryology, Histology); Foundational Neurosciences; Microbiology and Immunology; Physiologic Basis of Health and Disease (Pathophysiology); and Pharmacologic Principles and Concepts.

 What to Expect in the Final Exam

The COMAT FBS Comprehensive exam is a computer-based assessment. While specific forms may vary annually, students should expect a long-form examination, typically consisting of approximately 200 multiple-choice questions.

The examination is structured into two distinct sections, with a standard time limit of 4 hours total. Students generally have 2 hours to complete each 100-question section.

Specific rules include:

• Question Format: The questions are standard single-best-answer multiple-choice items.
• No Supplements: Calculators are not required and are not provided. There are currently no video or audio components within the COMAT series.
• Scoring: There is no negative marking for incorrect answers, so students are encouraged to answer every question. Scores are reported as standardized scores, often normed with a mean of 200 and a standard deviation of 10 for the comprehensive form, allowing for comparison across different test forms.

Each College of Osteopathic Medicine (COM) sets its own specific passing requirements, bearing weight on student grading and progression decisions.

 How to Study and Exam Centers

Effective preparation for the COMAT FBS requires a structured, long-term study plan that begins months before the test date. Because the exam covers the entirety of the pre-clinical curriculum, spaced repetition is vital for retention.

• Question Banks: Engaging with high-quality multiple-choice question banks specifically designed for osteopathic students (such as COMBANK or COMQUEST) is the most critical study method. This familiarizes you with the question style, vignette structure, and the integration of OPP. Aim to complete at least one full bank.
• Time Management: Practice timed blocks of questions to build endurance for the 4-hour format and to master a pace of approximately one minute per question.

Where and How to Take the Exam: COMAT examinations are institution-administered. They are not taken at external Pearson VUE centers. Your individual College of Osteopathic Medicine (COM) handles the registration, scheduling, and proctoring.

Passing the COMAT FBS is not a licensure certification that directly unlocks job titles. It is an academic milestone within medical school. However, success on this exam is the critical gateway to the clinical years of training and subsequent licensure as a Doctor of Osteopathic Medicine (DO).
