What the Course Entails and Exam Details

A New York State CFR course follows a rigorous curriculum standardized by the Department of Health (DOH) Bureau of EMS. It covers essential cognitive knowledge and psychomotor skills across several core divisions. The primary areas of focus include:

1. Preparatory: Introduction to EMS systems, the roles and responsibilities of the CFR, legal and ethical issues (consent, negligence, scope of practice), and the well-being of the first responder (wellness, stress management, BSI/PPE).

2. Airway: Management of the airway, artificial ventilation, and oxygen administration.

3. Circulation: Basic life support (CPR) including the use of an Automated External Defibrillator (AED), and management of foreign body airway obstruction. (Students must typically hold a separate AHA BLS Provider CPR or equivalent certification prior to state testing).

4. Patient Assessment: Methods of assessing medical and trauma patients, scene size-up, vital signs, and communications.

5. Illness and Injury: Recognition and management of general medical complaints (seizures, altered mental status, diabetic emergencies), bleeding, soft tissue injuries, and musculoskeletal trauma (fractures, spinal injuries).

6. Children and Childbirth: Management of emergency childbirth and care of the newborn, alongside specific assessment and treatment strategies for infants and children.

Additionally, candidates are required to complete several prerequisite certifications such as IS-100.C (Introduction to the Incident Command System) and IS-700.B (An Introduction to the National Incident Management System), as well as Hazardous Materials Awareness training.

What to Expect in the Final Exam

To become certified, you must successfully complete both parts of the official New York State Bureau of EMS certification examination process.

1. The State Practical Skills Examination: This is typically administered by a Course Sponsor near the end of your program. Candidates must demonstrate proficiency at several scenario-based skill stations. Mandatory stations include Patient Assessment-Trauma and Patient Assessment-Medical. There are also random skill stations testing various techniques like BVM ventilation or hemorrhage control. You must pass all stations to pass the practical exam. Failing three or more stations on the first attempt constitutes a complete failure of the practical examination. Failing one or two stations allows for re-tests.

2. The State Written Examination: This component tests cognitive knowledge of the CFR curriculum through multiple-choice questions. It is administered as a computer-based test (CBT) at authorized centers throughout New York State. The testing vendor currently utilized is PSI. A passing score of 70% or higher is required to gain certification. You must complete the written exam within one year of passing the practical skills exam. Remote proctoring options may also be available for the written examination.

3. Practice Scenario Flows: Practice patient assessment scenarios out loud with peers, visualizing the steps you must take and the information you need to gather. Use a stopwatch to ensure efficiency.

4. Utilize Study Guides and Mock Exams: Leverage provided course materials, review textbooks, and access reputable practice exam portals. Practice exams help you get comfortable with the multiple-choice format and identify areas needing more review.

5. Ensure All Prerequisites are Complete: Verify you have obtained your AHA BLS CPR, ICS 100/700, and HazMat Awareness certificates before testing, as these are mandatory course completion requirements.

Upon successful course completion and practical exam passage, you will receive an "Authorization to Test" (ATT) email from the state’s testing vendor, PSI. This email will contain instructions on how to create an account, schedule your written CBT exam at a location, date, and time that works for you. You can take the exam at a physical PSI testing center or explore the option for a Live Remote Proctored exam, subject to system compatibility checks.
