The final exam content is derived directly from the current Description of Specialty Practice (DSP).

What to Expect in the Final Exam

The actual OCS exam is not a simple walk-in. It is a comprehensive, full-day experience designed to challenge your stamina and your decision-making.

The standard final exam format is:

• Format: Computer-based multiple-choice exam, administered via high-security testing centers.

• Question Count: Typically consists of approximately 200 multiple-choice questions.

• Exam Structure: Divided into four sections, or blocks, each lasting 90 minutes. There are optional, scheduled breaks between each block.

• Case Studies: A significant portion of the exam consists of detailed clinical cases, requiring you to read a scenario, view clinical findings or images, and then answer a series of linked reasoning questions.

• Passing Score: The passing score is a scaled score determined by the American Board of Physical Therapy Specialties.

How to Study and Exam Centers

Preparation for the OCS should be viewed as a 6-12 month marathon, not a sprint.

Because of the heavy emphasis on clinical cases, memorizing facts will not be sufficient; you must understand the why behind clinical decisions.

Recommended Study Strategies:

• Master the DSP: The Description of Specialty Practice (DSP) should be your primary syllabus. Review it to understand exactly what is eligible for testing.

• Use Practice Exams: Take timed practice tests early and often. This builds test-taking endurance and helps identify your weakest subject areas.

• Focus on the ‘Why’: For every question, understand why the correct answer is right and why the distractors are wrong.

• Study Clinical Practice Guidelines (CPGs): CPGs for the major orthopedic conditions are crucial and frequently form the basis of clinical scenarios.

• Join a Study Group: Peer learning is invaluable for discussing complex reasoning scenarios.

Where to Take the Exam:

The OCS final exam is not administered at individual schools. You must take the exam at a professional testing center.

The ABPTS contracts with major, secure testing partners. Currently, this is completed through Prometric testing centers globally.

After completing your initial application and being deemed eligible by the ABPTS, you will receive an "Authorization to Test" which allows you to schedule your appointment at a physical Prometric testing location.
